DATA CONTROLLER AND APPLICATION METHOD
Personal data owners or their legal representatives (“Applicant” or “Relevant Person”) defined as the data subject within the scope of the Personal Data Protection Law No. 6698 (“Personal Data Protection Law” or “Law”), regarding the processing of personal data in Article 11 of the Personal Data Protection Law rights to make claims.
It is required to submit the applications in writing or other methods determined by the Personal Data Protection Board (“Board”) relating to these rights to Private Beyhekim Sağlık Hizmetleri San.ve Tic. Ltd. Şti. (“Beyhekim Hospital”) pursuant to the Article 13 of the Personal Data Protection Law. Depending on its nature, your request will be answered as soon as possible or within thirty days at the latest from the date it reaches Beyhekim Hospital. However, if the transaction requires a separate cost, a fee may be charged in accordance with Article 7 of the Communiqué.
APPLICATION METHOD
- Application in person or by mail: Application can be carried out in person or mail to Tepekum Mah, Beyhekim Sk. No:10 Adapazarı/Sakarya with the application form filled and signed by the Applicant and the documents proving his identity.
- Application Via Registered Electronic Mail (KEP): Application can be made by sending the application form to ozelbeyhekim@.hs.01kep.tr by signing with the “secure electronic signature” defined in the Electronic Signature Law No. 5070 by the applicant.
- Application via Electronic Mail Using Mobile Signature or Secure Electronic Signature:
An application can be made by sending an e-mail to info@beyhekimhastanesi.com.tr by filling out a petition signed by the Applicant with a mobile signature or secure electronic signature, or by filling in the “Application Form” if desired.
