DATA CONTROLLER AND APPLICATION METHOD

In accordance with the Personal Data Protection Law No. 6698 (“KVKK” or the “Law”), individuals identified as personal data owners or their legal representatives (“Applicant” or “Relevant Person”) are granted the right to make requests related to the processing of personal data as per Article 11 of KVKK.

According to the first paragraph of Article 13 of KVKK; the data controller, Özel Beyhekim Sağlık Hizmetleri San.ve Tic. Ltd Şti. (“Beyhekim Hospital”) requires that applications related to these rights be submitted to us in writing or through other methods determined by the Personal Data Protection Board (“Board”). Depending on the nature of your request, it will be answered as soon as possible or within a maximum of thirty days from the date it reaches Beyhekim Hospital. However, if the process involves any costs, a fee may be charged as per Article 7 of the Notification.

APPLICATION METHOD

• In-Person or Postal Application: The Applicant may apply personally or via post with a completed and signed application form and identity verification documents to: Tepekum Mah, Beyhekim Sk. No:10 Adapazarı/Sakarya, Address.

• Registered Electronic Mail (REM) Application: Applications can be made by sending a completed application form signed with a “secure electronic signature” as defined in the Electronic Signature Law No. 5070 to the address ozelbeyhekim@.hs.01kep.tr.

• Application via Email using Mobile Signature or Secure Electronic Signature: Applications can be made by emailing an application form or, if preferred, a completed “Application Form” signed with a mobile signature or secure electronic signature to bilgi@beyhekimhastanesi.com.tr.
